Forms Click each title below to download a copy of the following forms. Forms can either be printed blank or electronically filled in and printed or submitted online via email by clicking the Submit button at the bottom of the form. Forms that do not have a submit button at the bottom of the page must be printed and the original sent to the appropriate person. This especially applies to government forms that require original signatures, such as the W-4 form, and forms that require special attachments, such as the Direct Deposit Authorization forms. If you are unable to submit the form using the submit button, save the form (using Save As in the File menu) and email it as an attachment to the email address listed at the bottom of the form. If there is more than one email address listed, please be sure to include each address specified in your email. If you are not able to save the form, as many users of Acrobat Reader will not be able to, please print the form and mail or fax it to the appropriate person(s). If you have any questions or problems submitting or opening any of the forms, please call Sarah at the Central Office (287-2061) or contact Sarah via email at selton@esu1.org. You should receive some sort of notification, most likely by email, once your form has been received and processed. Please do not assume that your form has been processed immediately after submitting electronically. If you have not received some sort of notification after a reasonable amount of time, be sure to follow up with the appropriate department or contact person. Electronic submission constitutes agreement and signature.
