Surveys

ILCD surveys are available through ESU #1. If you would like to request surveys, please contact Wade Fruhling.

Instructions for ILCD Surveys

Please be advised that several survey forms have sections with special directions, which need to be followed. For example, if directions state to choose only one answer than please ONLY CHOOSE ONE.

Surveys will have the best scan results if:

• They are not folded.
• Nothing is stapled to the tic marks found on the left side of each survey.
• Additional comments are not added in the borders.
• Please NO pens - use pencils only.

Include a cover letter telling parents the primary disability of their child and any other demographic information. Another suggestion in helping parents understand the surveys would be to include a sheet explaining acronyms or other terms related to special education.
